Application for Advanced Degree

The process of applying for your advanced degree or certificate includes an Exit Survey, a List of Reminders, Required Information to Process Your Degree Completion, and a referral to the Alumni Association's site.

The Exit Questionnaire gathers data to help the Graduate School evaluate and improve our graduate programs. All information will be kept confidential and will be safeguarded in accordance with the Privacy Act of 1974 (as amended). Results from this questionnaire are reported in a manner that does not identify information about any individual. The information you supply is essential for future program planning.

A List of Reminders is provided to make sure you won't have surprises as you near the final stages of completing your degree.

The Required Information provides notification to the institution of your expectation for degree completion.

The Alumni Association record gives you the option of staying in touch with the university. You will receive the alumni magazine, Oregon Quarterly, as well as notifications of reunion activities planned by groups or organizations with whom you were affiliated while on-campus.
